Description of form: In order to protect all students and participants of activities, and to protect East Maiden Baptist Church, all participants under the age of 21 must have a completed medical release form on file. The form serves to provide information on current medications, injuries and allergies as well as contact information for parent(s)/guardian(s) in the event of an emergency. Also, it is requested that insurance information be provided as well.
Description of form: In order to maintain a Christian witness and a standard to which all students can adhere, our Student Ministry Rules & Dress Code Policies have been implemented. These policies outline what is and is not acceptable on ALL youth functions. These policies serve to benefit all involved as it eliminates areas of question.
